Description

Home Health/Hospice Registered Nurse (RN)

Location: Gunnison, Colorado
Employment Type: Contract
Contract Length: 13 weeks

Job Description

Provide patient care and case management using the nursing process and individualized Plan of Care for home health, hospice, and palliative care patients.
Administer skilled nursing care to patients requiring intermittent professional nursing services.
Conduct initial evaluation visits and regularly re-evaluate patients' nursing needs.
Initiate and revise plans of care as necessary.
Perform specialized nursing procedures and preventive/rehabilitative nursing interventions.
Prepare clinical and progress notes.
Coordinate services and communicate changes in patient condition to physicians and other personnel.
Counsel patients and families to meet nursing and related needs.
Participate in in-service programs and supervise/teach other nursing personnel.

Required Qualifications

Valid Registered Nurse (RN) license in Colorado
Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
Minimum 3 years of nursing experience
At least 1 year of home health nursing experience within the last year
Hospice nursing experience
Basic Life Support (BLS) certification
Colorado Adult Protective Services (CAPS) check required
Reliable transportation required due to limited public transportation in the rural area

Additional Information

Prior travel experience is required for this position.
