Description
Job Opportunity: Home Health RN
Location: Missoula, Montana
Employment Type: Contract
Contract Length: 13 weeks
Job Summary
Provide skilled nursing, care, and case management for patients in their place of residence or in an in-patient hospice setting. Responsibilities include admitting patients, coordinating and integrating services to ensure continuity of care, facilitating quality clinical and cost outcomes, and using the nursing process (assessment, planning, implementation, and evaluation) through an interdisciplinary team approach.
Essential Duties and Responsibilities
- Provide skilled nursing and coordinate care to patients and family members in a residential setting based on physician authorization.
- Observe, assess, evaluate, and plan interventions related to patients’ physical and psychosocial status, including monitoring changes in condition or reactions to treatments/medications.
- Oversee care provided by Home Health/Hospice Aides and CNAs and evaluate compliance with the plan of care.
- Ensure continuity of care through effective communication, information exchange, and case conferences with other disciplines and community support agencies.
- Provide quality care and symptom management to promote positive clinical outcomes and optimal health through teaching, counseling, interventions, and referrals.
- Maintain accurate, current, and complete patient records in accordance with agency standards and OASIS requirements.
- Participate in team scheduling coordination and on-call rotation as needed, including telephone assessments and home visits.
- Engage in quality improvement activities and attend in-service training.
Schedule
- Varies by setting:
- Typical in-home RN schedule: Monday to Friday, 8 AM to 5 PM
- Typical in-patient RN schedule: 3 rotating 12-hour shifts
Required Qualifications
- Valid Registered Nurse (RN) license in the State of Montana
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Minimum 1 year of Home Health nursing experience, including experience within the last year
- Experience in a Home Care setting
- Acute Care nursing experience
- Hospice nursing experience
