Job Opportunity: LVN / LPN - Home Health (Travel/Contract)

Location: Georgetown, Delaware
Contract Length: 13 weeks


Job Description

  • Provide and coordinate home nursing care according to physician orders and agency policies.
  • Direct supervision and evaluation of Home Health Aides and Licensed Practical/Vocational Nurses.
  • Average patient load: 5-8 patients per day.
  • Types of visits: Start of Care, Wound Care, Follow-up Visits, etc.
  • Must be willing to travel up to 75 miles from home clinic to patient homes.
  • Use of electronic medical records system: PointClickCare.
  • Schedule: Monday to Friday, 8 AM to 5 PM (most assignments). Some locations may have a 4x10 shift (Friday-Monday).
  • No guaranteed hours; flexibility to float to nearby clinics if patient census is low.
  • Local candidates accepted at the same rate.

Required Qualifications

  • Valid LPN / LVN license in Delaware.
  • Minimum 1 year of recent Home Health experience within the last year, specifically working with adult patients.
  • BLS certification issued by the American Heart Association or American Red Cross.
  • Experience in a Home Health setting.

Travel & Work Details

  • Territory: Sussex County, Delaware.
  • Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable).
  • Home visits with periodic office meetings.

Compliance & Documentation Requirements

  • Must complete standard background checks including 7-year history, sex offender, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable.
  • Fingerprinting required prior to start (state-specific rules apply).
  • Education verification required.
  • Physical exam within 12 months confirming good health and no communicable diseases.
  • Negative TB test or chest X-ray within 12 months, updated annually.
  • Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
  • Drug screening: 10-panel drug test required.
  • Seasonal flu vaccination or documented declination required.
  • COVID-19 vaccination series or declination required.
  • Two professional references required.
  • Verification of work history for at least the last 7 years or two most recent employers.
  • Must review and sign various compliance, confidentiality, privacy, and policy documents prior to start.

Additional Notes

  • Candidates must be willing to complete all required documentation and compliance training prior to assignment start.
  • Candidates must have reliable transportation and insurance coverage for travel to patient homes.
  • This is a travel/contract position with no guaranteed hours; flexibility and adaptability are important.

This position is ideal for experienced LVNs/LPNs with recent home health or hospice experience who are comfortable working independently in a home care setting and traveling within a defined territory.
