Job Opportunity: Physical Therapist - Home Health
Location: Medford, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Provide skilled physical therapy services in a home health setting.
- Work 8-hour day shifts (no 4x10 or 3x12 shifts available).
- Patient ratio is 1:1, with up to 6 patient visits per day.
- Productivity standard: 30 points per day.
- Must have own transportation; occasional travel up to 50+ miles.
- Facility uses EPIC and Oasis for charting.
- Scrubs of any color are acceptable.
- Free parking available.
- Mileage reimbursed at IRS standard rates.
- Facility requires primary series COVID vaccination plus one booster.
- Oasis experience is required; please disclose if not experienced.
Required Qualifications
- Valid Physical Therapist license in Oregon.
- Minimum of 1 year of Physical Therapist experience within the last year.
- Experience in a Home Health setting.
- Experience with OASIS documentation.
- Completion of at least one travel assignment.
Additional Information
- No weekend shifts required.
- No on-call or floating between units.
- Locals within 50-75 miles considered local and accepted.
- Facility may call off up to one shift per pay period.
- Traveler must comply with all state-specific background checks and licensing requirements.
- Must complete required WorkRamp training modules annually.
- Fit mask testing required for travel candidates.
- Respiratory safety clearance required annually for travel candidates.
- Physical exam must indicate ability to work with or without accommodation.
- Facility requires completion of Oregon Neglect Abuse Search if applicable.
- Must comply with all facility and state-specific documentation and credentialing requirements.
