Job Opportunity: Physical Therapist - Home Health

Location: Medford, Oregon

Job Description

• Provide skilled physical therapy services in a home health setting.
• Work 8-hour day shifts (no 4x10 or 3x12 shifts available).
• Patient ratio is 1:1, with up to 6 patient visits per day.
• Productivity standard: 30 points per day.
• Must have own transportation; occasional travel up to 50+ miles.
• Facility uses EPIC and Oasis for charting.
• Scrubs of any color are acceptable.
• Free parking available.
• Mileage reimbursed at IRS standard rates.
• Facility requires primary series COVID vaccination plus one booster.
• Oasis experience is required; please disclose if not experienced.

Required Qualifications

• Valid Physical Therapist license in Oregon.
• Minimum of 1 year of Physical Therapist experience within the last year.
• Experience in a Home Health setting.
• Experience with OASIS documentation.
• Completion of at least one travel assignment.

Additional Information

• No weekend shifts required.
• No on-call or floating between units.
• Locals within 50-75 miles considered local and accepted.
• Facility may call off up to one shift per pay period.
• Traveler must comply with all state-specific background checks and licensing requirements.
• Must complete required WorkRamp training modules annually.
• Fit mask testing required for travel candidates.
• Respiratory safety clearance required annually for travel candidates.
• Physical exam must indicate ability to work with or without accommodation.
• Facility requires completion of Oregon Neglect Abuse Search if applicable.
• Must comply with all facility and state-specific documentation and credentialing requirements.
