Physical Therapist (PT) - Long Term Care (Days)

Location: Lewistown, Missouri
Employment Type: Temporary
Contract Length: 13 weeks

Job Description

• Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care.
• Develop and implement appropriate plans of care following regulatory and clinical practice standards.
• Utilize standardized assessments and evidence-based practices to support clinical interventions.
• Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Assess patient progress and adjust treatments accordingly, documenting in clinical records per regulatory requirements.
• Evaluate treatment outcomes and modify services as needed; make referrals based on outcomes.
• Provide consultation and counseling to patients, families, caregivers, and other service providers.
• Communicate effectively with supervisors and healthcare team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summaries per regulatory and clinical requirements.
• Document all required information accurately in electronic systems and maintain patient records according to policy.
• Record treatment minutes in compliance with regulatory and clinical standards.
• Participate in required meetings such as patient care conferences, utilization review, and rehabilitation meetings.
• Attend facility and company-required trainings, in-services, and conferences.
• Obtain necessary durable medical equipment (DME) or supplies as required.
• Supervise and support physical therapist assistants, rehabilitation aides, and/or students.
• Adhere to APTA and state-required practice acts, code of conduct, and ethics during professional practice.

Required Qualifications

• Minimum 1 year of Physical Therapist experience.
• Experience in Long Term Care (LTC) or Rehabilitation settings is required.
• Missouri State Physical Therapist License required.
• New graduates are acceptable if they have completed Level II clinical experience (not just Level I observation).

Additional Information

• Standard 8-hour day shift.
• Participation in clinical interview process with timely outreach and decision-making.
• Candidates seeking more than 7 days off or more than 50% of holidays off, or with negative background/employment history, may not be eligible for expedited processing.
