Outpatient Physical Therapist
Location: Northampton, Massachusetts
Employment Type: Long-Term Assignment (13 weeks)
Shift:
- Monday, Wednesday, Friday: 8:30 AM – 5:00 PM
- Tuesday, Thursday: 9:30 AM – 6:00 PM
Job Description
Provide patient care focused on rehabilitation for individuals with special needs to optimize their physical, functional, psychological, social, and emotional well-being.
Required Qualifications
- Valid Physical Therapist license in Massachusetts
- Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross
Experience
- Specialty: Physical Therapist Outpatient
- Required years of experience: Not explicitly stated; assume entry-level or as per license requirements

Onboarding and Compliance Requirements
Professional Credentials
- Submission of a recent (within 6 months) color badge photo (no selfies, plain background, no eyeglasses)
- Resume including past 7 years of work experience with city and state
- Two professional references within the last 4 years (minimum one supervisor or above in relevant skillset)
- Skills checklist completed within 1 year of submission
- Confirmation of Assignment/POI form upon acceptance and for any start date changes
Licensure and Background Checks
- Primary source verification of professional license and certifications within 30 days of start date
- Criminal background check within 45 days prior to start date, including county and international residence/employment history for the last 7 years
- Massachusetts CORI background check within 30 days prior to start date
- Social Security Number trace for the past 7 years
- National sex offender registry check within 30 days prior to start date and updated annually
- Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days prior to start and annually
- Education verification of highest degree achieved
Health and Immunization Requirements
- Documentation of immunity or vaccination for:
- Measles, Mumps, Rubella (two MMR vaccines or proof of immunity)
- Tuberculosis screening within 3 months of start date (skin test or blood test) and updated if break >6 months
- Physician health statement signed by MD/DO/NP/PA within 365 days of start date and updated if break >6 months
- Hepatitis B vaccination or proof of immunity strongly preferred (optional)
- Tdap vaccination within 10 years preferred (optional)
- If positive TB test, documentation of negative chest x-ray required (optional)
Additional Notes
- Contract length is 13 weeks.
- No prior travel experience required.
- This position requires compliance with multiple background and health screenings prior to start.
- Work schedule includes some variation in start/end times on different days.
