Job Opportunity: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Home Health
Location
- City: Winder, Georgia
- Travel radius: Must be willing to travel up to 75 miles from home clinic
- Patient visits include both home and care facilities
Contract Details
- Type: Travel/Contract
- Length: 13 weeks
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hour shifts)
- Average patients per day: 5-8
- Weekly travel: Approximately 50-60 miles
- On-call: 1-2 weeknights per week (from office closing until next morning)
- Locals accepted at the same rate
Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies
- Supervise and evaluate Home Health Aides and other Licensed Practical/Vocational Nurses
- Types of visits: Start of Care, Wound Care, Follow-up Visits, etc.
- Documentation and charting required using HomeCare HomeBase (HCHB); proficiency required within last 6 months or retraining provided
- Must be proficient with Outlook; RNs/PTs/OTs also require proficiency in OASIS
Required Qualifications
- Valid LPN / LVN license in Georgia
- Minimum 1 year of recent experience in Home Health, including experience working with adult patients
- Hospice experience
- Proficiency with HomeCare HomeBase EMR system within the last 6 months
Additional Requirements
- Must complete standard background checks including 7-year history, sex offender, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable
- Drug screening: 10-panel required
- Physical exam within 12 months confirming good health and no communicable diseases
- Negative TB test or chest X-ray within 12 months (no pending results)
- Immunization or titer proof for Measles, Mumps, Rubella, Hepatitis B, Varicella as per standard requirements
- COVID-19 vaccination series or declination required
- Flu vaccination or declination documentation required for current season
- Two professional references required
- Verification of work history for at least the last 7 years or two most recent employers

- HIPAA and Privacy Policy
- Confidentiality Form and Covenant
- Compliance Training Certification
- Home Health Scenarios and Core Policy Acknowledgements
- COVID-19 Policy and Attestation
- Employee Responsibility Agreement
- TB Questionnaire
- Must maintain up-to-date personal or auto insurance policy covering travel for patient visits
Notes
- Weekend and on-call requirements vary; discuss during interview
- Schedule may vary in some territories (e.g., 4x10 shifts Friday-Monday possible)
- No guaranteed hours; patient census may fluctuate
- Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed)
