Occupational Therapist - Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (13 weeks)
Shift: 8:00 AM – 4:30 PM
Job Summary
Provide occupational therapy services to patients in their homes who have cognitive and/or physical impairments due to injury, disease, disability, or other causes. Patient age range is from birth to over 100 years. Collaborate with patients, families, physicians, and allied health professionals to assess needs and coordinate care. Manage technical, environmental, and financial resources related to patient care. Travel within the geographic area is required.
Required Qualifications
- Valid Occupational Therapist license in Massachusetts
- Minimum 2 years of Occupational Therapist experience, including experience within the last year
- Experience in Acute Care, Home Care, and Rehabilitation settings
Travel Experience
- Travel within the assigned geographic area is required.
- No explicit prior travel experience requirement stated.
Required Documentation and Compliance
- Professional license/certification primary source verification (no conditional licenses accepted).
- Resume including past 7 years of work experience with city and state details.
- Two professional references within the last 4 years (minimum one supervisor-level).
- Skills checklist completed within 1 year of submission.
- Criminal background check, including Massachusetts CORI, national sex offender registry, and exclusion checks (OIG, GSA, Medicaid lists).
- Motor Vehicle Record (MVR) check within 45 days of start date.
- Education verification of highest degree achieved.
- Social Security Number trace for the past 7 years.
- Confirmation of Assignment/POI form upon acceptance and for any assignment changes.
Health and Safety Requirements
- N95 Fit Test within 365 days of start date, updated annually (no exemptions allowed).
- Respirator Medical Clearance attestation if fit test required.
- Documentation of TB screening within 3 months of start date and updated if there is a break >6 months (skin test or blood test).
- Physician health statement signed by MD/DO/NP/PA within 365 days of start date, updated if >6 month break.
- Documentation of MMR immunity or vaccination.
- TB symptom survey if positive TB test (optional but recommended).
- Hepatitis B vaccination or immunity strongly preferred.
- Tdap vaccination within 10 years preferred.
- Positive TB test requires negative chest x-ray documentation (optional).
Additional Notes
- Must submit a professional badge photo (color, plain background, no filters or eyeglasses).
- Name change documentation required if legal name differs from credential documents.
