Mental Health Counselor
Location: Gardner, Massachusetts
Employment Type: Contract
Contract Length: 13 weeks
Job Description
- Systematically gather and evaluate relevant patient information from interviews and other sources.
- Ensure patient exams, tests, and procedures align with patient needs and orders.
- Identify and report variations from expected findings to the charge nurse.
- Provide patient education and involve family/significant others when appropriate.
- Prioritize requests based on patient condition and situation.
- Assess and reassess patient conditions regularly.
- Coordinate and communicate with healthcare professionals to deliver comprehensive care.
- Delegate tasks appropriately within the healthcare team.
- Support case management to meet standards of care and practice.
- Collect patient information to develop comprehensive treatment plans.
- Coordinate care with primary care providers, outpatient providers, and other services.
- Participate in multidisciplinary treatment planning.
- Respond to assigned case management duties including diagnostics, treatment planning, and disposition.
- Identify insurance requirements and obtain necessary approvals or referrals.
- Provide individual and group therapies according to Mental Health Unit standards.
- Document interventions and patient responses per policy.
- Support supervision of students and interns to ensure performance and documentation standards.
- Pursue professional growth and share knowledge with team members.
Required Qualifications
- Valid Mental Health license in Massachusetts
- Licensed Clinical Social Worker (LCSW)
- Bachelor of Arts or Bachelor of Science degree in Psychology, Human Services, or a related field
- Minimum 1 year of clinical experience in the Human Services field
- Minimum 1 year of mental health experience, including experience within the last year
