Description

Physical Therapist (PT) - Contract Position

Location: Grand Junction, Colorado
Employment Type: Contract
Contract Length: 13 weeks


Job Overview

  • Provide physical therapy services for elderly participants in a comprehensive care program aimed at maintaining and improving seniors' health to support safe living at home.
  • Conduct initial, annual, and condition-change assessments for participants.
  • Collaborate with an interdisciplinary team, participants, and families to develop and update individualized plans of care (POC).
  • Deliver clinical education and training to staff and volunteers to enhance participant safety and functional abilities during activities of daily living (ADLs).

Work Schedule

  • Monday to Friday, 8:00 AM to 5:00 PM

Required Qualifications

  • Valid Physical Therapist license in Colorado
  • Minimum of 1 year of Physical Therapist experience within the last year
  • Current Basic Life Support (BLS) certification

Skills and Competencies

  • Proficient in Microsoft Word, Excel, Access, and Outlook.
  • Excellent interpersonal and communication skills, able to communicate effectively and respectfully in various situations, including emotionally charged interactions.
  • Ability to work effectively with others and build strong professional relationships.
  • Comfortable working in diverse environments.
