Description

Hospice Registered Nurse (Home Hospice Care)

Job Type

  • Contractor
  • Contract length: 13 weeks

Work Setting

  • Home Hospice RN position providing care in patients’ homes
  • Quarterly in-person team meetings at a regional hospital
  • Initial supply pickup at regional office
  • Coverage territory includes multiple suburban towns

Patient Population

  • Adults with diagnoses such as failure to thrive, advanced cancers, end stage COPD, terminal illness, and other end stage disease processes
  • Average caseload: 12-15 patients

Schedule

  • Monday to Friday, 8:00 AM – 4:30 PM
  • No weekends, holidays, or on-call duties
  • Set schedule with expected work until shift end

Required Qualifications

  • Minimum 3 years of Hospice Palliative Care RN experience, including experience within the last year
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • BLS certification
  • Valid driver’s license, auto insurance, and reliable vehicle
  • Home Health nursing experience

Skills and Responsibilities

  • Wound care including dressing changes and wound vac management (wounds must be photographed and measured weekly)
  • Care of drains including nephrostomy care, Pleurex, and Foley
  • End of life care and planning
  • Pain and symptom management
  • Patient and family education
  • Triage and care coordination
  • Strong organizational and time management skills

Orientation and Support

  • New Employee Orientation (NEO) lasting 1-2 weeks, adjusted based on experience and comfort level
  • Provided with phone, laptop, and trunk stock when on site
  • Access to other hospice RNs and management via phone for support

Technology

  • Electronic Medical Record system: EPIC Remote Client

Additional Information

  • Scrub color: Navy blue or any solid professional color without graphics, logos, or designs
  • Free parking available

Travel Experience

  • First-time travelers accepted (prior travel experience not required)
