Job Opportunity: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Shift Details
- Evening Shift: 3:00 PM – 11:30 PM (with possible flexibility)
- Night Shift: 11:00 PM – 7:30 AM (possible openness to nights)
- Hours per day: 8
- Hours per week: 40
- Weekend Coverage: Every other weekend (total of 8 weekend shifts during contract)
Patient Population & Unit Information
- Mix of Long Term Care and Rehabilitation patients
- Float required between all 3 units in the building
- Average daily census: 120 patients
- Nurse-to-patient ratios:
- Days: 1:20
- Nights: 1:40
- Weekends: 1:20
- Unit accepts first-time travelers
Required Qualifications
- Valid LPN / LVN license in New York
- Minimum 1 year of Long Term Care experience within the last year
- BLS certification issued by the American Heart Association
- Experience in a Long Term Care setting
Required Documentation & Compliance
- Government-issued photo ID (Driver’s License or State ID)
- Background checks including:
- 7-year criminal background check (felony checks)
- EPLS/GSA/SAM and HHS/OIG checks (within 3 months of start, run monthly)
- Education verification from primary source (required for non-CNA positions)
- Foreign equivalency education evaluation if applicable
- State-specific requirements:
- New York Nurse Aide Registry verification (within 3 months of start for LTC)
- Fingerprinting and criminal background checks as required for LTC units
- Sex Offender Registry search within 30 days of start
- Medicaid Exclusion List check (within 3 months of start, run monthly)
Health & Safety Requirements (Must be completed within 5 days of contract signing)
- Physical exam (within 1 year, updated annually)
- Fit mask test (annual; acceptable masks specified)
- Negative TB test or chest X-ray (within 1 year)
- TB questionnaire (in lieu of annual TB test renewal)
- Immunizations or proof of immunity:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (surface antibody positive titer, completed series, or declination)
- Varicella
- Drug screening (within 30 days of start)
- Seasonal flu vaccination or signed declination (required during flu season)
- Snellen eye exam (within 1 year)
- COVID vaccination copy preferred but not required
Competency & Training Requirements
- Medication test/pharmacology competency exam as required
- Color blind testing (one-time)
- Other competency exams as required by specialty
- Orientation safety manual acknowledgement
- State license primary source verification within 30 days of start
- NURSYS verification for LPN license within 30 days of start
Additional Notes
- Scrub color: Any color acceptable
- References: Required only for certain direct hire candidates (2 references within 3 years)
- Employment application and work history verification required
- Skills checklist required annually
This position offers a structured LTC travel nursing opportunity with a defined schedule, clear patient ratios, and comprehensive compliance and health requirements.
