Description

Job Opportunity: Home Health/Hospice Registered Nurse (RN)

Location: Gunnison, Colorado
Employment Type: Contract
Contract Length: 13 weeks


Job Description

  • Provide patient care and case management for home health, hospice, and palliative care patients.
  • Conduct initial evaluation visits and regularly re-evaluate patients' nursing needs.
  • Initiate and revise plans of care as necessary.
  • Deliver specialized nursing services and preventive/rehabilitative nursing procedures.
  • Prepare clinical and progress notes.
  • Coordinate services and communicate changes in patient condition to physicians and other personnel.
  • Counsel patients and families regarding nursing and related needs.
  • Participate in in-service training programs.

Work Schedule

  • Day shift.
  • On-call requirements: 1-2 nights per week and 1-2 weekends per month.
  • Approximately 36 hours per week.

Required Qualifications

  • Valid Registered Nurse (RN) license in Colorado.
  • Basic Life Support (BLS) certification.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Minimum 1 year of hospice experience within the last year.
  • Home health nursing experience.

Additional Information

  • This position involves working in a rural setting.
  • Prior travel experience is required.
