Location: Dover, New Hampshire
Assignment Length: 8 weeks
Shift: Day shift, 4x10 hours (06:45 AM – 5:15 PM)
Employment Type: Long-term assignment (LTA)
Job Description
- Operating Room Registered Nurse position
- 4 ten-hour day shifts per week
- Local candidates will not be considered
Required Qualifications
- Valid RN license in New Hampshire
- Minimum of 1 year of Operating Room (OR) experience within the last year
- Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross
- Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
Health and Compliance Requirements
- Documentation of immunity or vaccination for:
- Measles, Mumps, Rubella (two MMR vaccines or proof of immunity via blood test)
- Tuberculosis (TB) screening:
- TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start date
- If positive TB test, documentation of a negative chest x-ray after positive test required
- Health statement signed by MD/DO/NP/PA within 1 year of start date
- N95 Respirator Fit Test within 1 year of start date, updated annually (no exemptions allowed)
- Respirator Medical Clearance attestation required if fit test is required
Background and Credentialing Requirements
- Criminal background check within 45 days prior to start date, including county and international residence/travel history for last 7 years
- New Hampshire state-specific criminal background and BAAS checks within 30 days prior to start date
- National sex offender registry check within 30 days prior to start date, updated annually
- Exclusion checks (OIG, GSA, Medicaid lists) within 30 days prior to start date, updated annually
- Social Security Number trace for last 7 years addresses and aliases
- Verification of highest level of education achieved (HS/AS/BA/MA/advanced degree or certification)
- Resume including past 7 years of work experience with city and state
- Two professional references within 4 years, including at least one supervisor or higher
- Skills checklist completed within 1 year of submission
- Confirmation of assignment/POI form required upon acceptance and for extensions
Additional Notes
- Fit testing cannot be performed onsite; must be completed prior to assignment start
- Emergency NH license applicants should apply for emergency license within 48 hours of permanent license application
- Tdap vaccine within 10 years preferred but not required
- Hepatitis B vaccination or immunity strongly preferred but optional
- Name change documentation required if legal name differs from credential documentation
