Job Opportunity: Home Health LVN / LPN

Location: Chesapeake, Virginia
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
• Average patient load: 5-8 patients per day.
• Types of visits include Start of Care, Wound Care, and Follow-up Visits.
• Must be willing to travel up to 75 miles from the home clinic; average weekly travel approximately 200 miles.
• Reliable transportation, current driver’s license, and agency-required liability insurance are mandatory.
• Charting system: Proficiency with HomeCare HomeBase (HCHB) required, with recent use within the last 6 months or willingness to retrain.
• Dress code: Scrubs.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours). Weekend and on-call requirements vary and should be discussed during the interview.

Required Qualifications

• Valid LPN / LVN license in Virginia.
• Minimum 1 year of recent Home Health experience, including experience within the last year.
• Experience working with adult patients in a Home Health setting.
• Hospice experience.
• BLS certification issued by the American Heart Association or American Red Cross.

Additional Requirements

• Must complete a comprehensive background check including:

  • 7-year history verification of locations lived/worked/schooled.
  • State-specific searches and registries as applicable.
  • Fingerprinting as required by state regulations.
• Required health documentation:

  • Physical exam within 12 months prior to start.
  • Negative TB test or chest X-ray within 12 months.
  • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
  • Drug screening (10-panel).
  • Seasonal flu vaccination or documented declination.
  • COVID-19 vaccination series or declination.
• Must have valid personal or auto insurance policy under candidate’s name or list candidate as a driver.
• Must complete various compliance, confidentiality, privacy, and policy acknowledgments prior to start.

Work Environment & Expectations

• Home visits with periodic office meetings.
• Patient-to-patient travel is compensated; commute to/from first and last patient is not billable.
• Productivity expectations: approximately 32 weekly units.
• No guaranteed hours; workload may fluctuate. If hours are insufficient, floating to a nearby clinic may be arranged.
