Job Opportunity: Home Health LVN / LPN
Location: Chesapeake, Virginia
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
- Average patient load: 5-8 patients per day.
- Types of visits include Start of Care, Wound Care, and Follow-up Visits.
- Must be willing to travel up to 75 miles from the home clinic; average weekly travel approximately 200 miles.
- Reliable transportation, current driver’s license, and agency-required liability insurance are mandatory.
- Charting system: Proficiency with HomeCare HomeBase (HCHB) required, with recent use within the last 6 months or willingness to retrain.
- Dress code: Scrubs.
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours). Weekend and on-call requirements vary and should be discussed during the interview.
Required Qualifications
- Valid LPN / LVN license in Virginia.
- Minimum 1 year of recent Home Health experience, including experience within the last year.
- Experience working with adult patients in a Home Health setting.
- Hospice experience.
- BLS certification issued by the American Heart Association or American Red Cross.
Additional Requirements
- Must complete a comprehensive background check including:
- 7-year history verification of locations lived/worked/schooled.
- State-specific searches and registries as applicable.
- Fingerprinting as required by state regulations.
- Required health documentation:
- Physical exam within 12 months prior to start.
- Negative TB test or chest X-ray within 12 months.
- Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
- Drug screening (10-panel).
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or declination.
- Must have valid personal or auto insurance policy under candidate’s name or list candidate as a driver.
- Must complete various compliance, confidentiality, privacy, and policy acknowledgments prior to start.
Work Environment & Expectations
- Home visits with periodic office meetings.
- Patient-to-patient travel is compensated; commute to/from first and last patient is not billable.
- Productivity expectations: approximately 32 weekly units.
- No guaranteed hours; workload may fluctuate. If hours are insufficient, floating to a nearby clinic may be arranged.

- Two professional references required.
- Verification of at least two most recent employers or last 7 years of work history.
This position is ideal for experienced LVN/LPNs with recent home health or hospice experience seeking a travel contract role in Virginia with a structured day shift schedule.
