Job Opportunity: Home Health LVN / LPN
Location: West Bridgewater, Massachusetts
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits with an average patient load of 5-8 patients per day.
- Types of visits include Start of Care, Wound Care, and Follow-up Visits.
- Must have reliable transportation and a current driver’s license.
- Willingness to travel up to 75 miles from the home clinic; approximately 250 miles traveled per week.
- Use of HomeCare HomeBase (HCHB) EMR system required; proficiency within the last 6 months mandatory.
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours). Weekend rotation and on-call requirements vary and should be discussed during the interview.
- Dress code: Scrubs.
Required Qualifications
- Valid LPN / LVN license in Massachusetts
- Minimum 1 year of recent Home Health experience within the last year
- Experience working with adult patients in a Home Health setting
- Current BLS certification issued by the American Heart Association or American Red Cross
Additional Requirements
- Must have reliable transportation, current driver’s license, and agency-required liability insurance.
- Ability to manage documentation, travel, and patient care activities that contribute to productivity expectations (32 points per week).
- Willingness to discuss weekend and on-call requirements during the interview as they vary by location.
Compliance and Documentation
- Background checks including 7-year history, sex offender, FACIS, EPLS/GSA/SAM, and state-specific searches as required.
- Fingerprinting required per state regulations (varies by state).
- Education verification and work history verification (minimum 2 most recent employers or last 7 years).
- Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella, and seasonal flu vaccination or declination.
- Drug screening: 10-panel drug test required.
- COVID-19 vaccination series or declination required.
- Completion and signing of various compliance documents including confidentiality, HIPAA, privacy policies, and home health-specific policies prior to start.
Travel and Local Candidates
- Local candidates accepted at the same rate.
- Must be willing to travel within a 75-mile radius from the home clinic location.
Notes
- Some positions may follow a Baylor shift schedule (4x10s Friday-Monday), but most are standard 5x8 Monday-Friday.
- Weekend and on-call requirements vary by location and should be clarified during the interview process.
- Productivity expectations are based on a point system related to patient visits and documentation.
This position is ideal for experienced LVN/LPNs with recent home health or hospice experience seeking a travel contract role in Massachusetts.
