Description

Registered Nurse - Home Health (Travel/Contract)

Location: North Charleston, South Carolina
Contract Length: 13 weeks
Schedule:

  • Monday to Friday: 8:00 AM – 5:00 PM
  • Thursday to Sunday: 8:00 AM – 5:00 PM
  • On-call: Saturday and Sunday, 8:00 PM – 5:00 AM

Job Description

  • Provide and coordinate home nursing care according to physician orders and agency policies.
  • Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
  • Conduct home visits and attend periodic office meetings.
  • Average patient load: 5-8 patients per day.
  • Types of cases include Start of Care, Wound Care, and Follow-up Visits.
  • Must be willing to travel up to 75 miles from the home clinic; travel is compensated from patient home to patient home (commute to/from first and last patient is not billable).
  • Use of electronic medical records systems: HomeCare HomeBase (HCHB), OASIS, and Outlook.
  • Dress code: Scrubs.

Required Qualifications

  • Valid Registered Nurse (RN) license in South Carolina.
  • Minimum of 1 year of recent experience in Home Health nursing, including experience with adult patients.
  • Experience in Hospice care.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Current BLS certification issued by the American Heart Association or American Red Cross.
  • Proficiency with HomeCare HomeBase (HCHB) and OASIS documentation systems.

Additional Requirements

  • Must complete a variety of background checks and verifications including:
    • 7-year standard background check with state-specific searches.
    • Fingerprinting (required if not residing in South Carolina for 12 months prior to start).
    • Drug screening (10-panel).
    • Education verification.
    • References: Two required for all placements.
  • Health documents required prior to start and updated annually:
    • Physical exam within 12 months confirming good health and no communicable diseases.
    • Negative TB test or chest X-ray within 12 months.
    • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
    • Seasonal flu vaccination or documented declination.
    • COVID-19 vaccination series or declination.
  • Must review and sign various compliance, confidentiality, privacy, and policy documents prior to start and annually thereafter.

Work Environment & Expectations

  • Patient visits occur in both home and care facility settings.
  • Weekend and on-call requirements vary; discuss during interview.
  • Schedule may include standard 5x8 shifts or, in some territories, 4x10 shifts (Friday-Monday).
  • No guaranteed hours; if patient census is low, floating to nearby clinics may be arranged.
