Speech and Language Pathologist (SLP) - Temp, Long Term Care (LTC)
Location: Ada, Oklahoma
Employment Type: Temporary
Shift: Days
Hours per Day: 8 hours
Contract Length: 13 weeks
Job Description
- Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders.
- Develop and adjust individualized plans of care using standardized assessments and evidence-based practices.
- Assess patient progress and modify treatments accordingly, documenting all clinical and regulatory information in electronic records.
- Evaluate treatment outcomes and make referrals as needed.
- Provide consultation and counseling to patients, families, caregivers, and other service providers.
- Communicate effectively with supervisors and healthcare team members regarding patient progress and treatment plans.
- Prepare comprehensive discharge summaries per regulatory and clinical requirements.
- Accurately record treatment minutes in compliance with all regulations.
- Participate in patient care conferences, utilization review meetings, rehabilitation meetings, and required trainings or in-services.
- Obtain necessary durable medical equipment (DME) or supplies as required.
- Supervise and support Clinical Fellowship Year (CFY) or Student Speech-Language Pathologists.
- Adhere to ASHA and State practice acts, code of conduct, and ethics.
Required Qualifications
- Minimum 1 year of Speech-Language Pathology experience.
- Experience in Long Term Care (LTC) or Rehabilitation settings.
- Active Oklahoma State License.
- Certification of Clinical Competence (CCC) from ASHA.
- New graduates are eligible if they have completed Level II experience (beyond Level I observation).
