Description

Registered Nurse Diabetes Educator

Location: Rockford, Illinois
Employment Type: Contract (13 weeks)
Shift: Monday to Friday, Day Shift (40 hours/week)


Job Summary

Provide compassionate, culturally sensitive, holistic nursing care focused on diabetes education for patients and families living with diabetes mellitus. Collaborate with a multidisciplinary diabetes management team to develop and execute individualized patient care plans. May float to various units based on clinical competency and departmental needs.


Essential Duties and Responsibilities

  • Utilize the nursing process and collaborate with interdisciplinary team members in patient care.
  • Administer prescribed medications and treatments following nursing standards, policies, and state laws.
  • Document care according to standards.
  • Develop, initiate, and execute individualized patient care plans, including patient and family education.
  • Participate in department/unit performance improvement and shared governance activities.
  • Provide clinical expertise and guidance to other units/departments supporting patient care continuity.
  • Apply critical thinking to ensure patient safety and experience; escalate issues appropriately.
  • Act as a resource for staff regarding patient education programs and materials.
  • Advocate for patients addressing clinical, psychosocial, and financial issues.

Required Qualifications

  • Valid Registered Nurse (RN) license in Illinois
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Certified Diabetes Care and Education Specialist (CDCES) certification
  • Basic Life Support (BLS) certification
  • Minimum of 2 years of experience as a diabetes educator
  • Diabetes educator experience within the last year

Additional Information

  • Work schedule is Monday through Friday, day shift.
  • May include variable shifts, weekends, holidays, and/or on-call hours as needed.
  • Ability to float to various units based on clinical competency.

This position is ideal for a nurse with diabetes education certification seeking a contract role focused on patient education and interdisciplinary collaboration.
