Job Opportunity: Home Health Nurse (LVN / LPN)

Location: Marlborough, Massachusetts
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Vocational Nurses.
• Average patient load: 5-8 patients per day.
• Types of visits include Start of Care, Wound Care, and Follow-up Visits.
• Must be willing to travel up to 75 miles from the home clinic; average weekly travel approximately 250 miles.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours). Weekend rotation and on-call requirements vary and should be discussed during the interview.
• Dress code: Scrubs.
• Charting system: Proficiency required in HomeCare HomeBase (HCHB), with recent use within the last 6 months or willingness to retrain.

Required Qualifications

• Valid Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) license in Massachusetts.
• Minimum of 1 year of experience in Home Health, including experience within the last year.
• At least 1 year of total travel nursing experience.
• Experience working with adult patients in a Home Health setting.
• Hospice experience.
• Current BLS certification issued by the American Heart Association or American Red Cross.
• Proficiency with HomeCare HomeBase (HCHB) electronic medical record system.

Additional Requirements & Compliance

• Must complete a variety of background checks including:

  • 7-year standard background check covering all aliases and locations.
  • State-specific searches including Massachusetts CORI.
  • Sex offender registry checks.
  • EPLS/GSA/SAM and HHS/OIG screenings.
  • Fingerprinting as required by state regulations.
• Education verification for applicable degree.
• Health requirements include:

  • Physical exam within 12 months prior to start.
  • Negative TB test or chest X-ray within 12 months.
  • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
  • 10-panel drug screen.
  • Seasonal flu vaccination or documented declination.
  • COVID-19 vaccination per Massachusetts state policy (no declinations accepted).
• Must review and sign various compliance, confidentiality, and policy documents prior to start.
• Two professional references required.
• Verification of work history for at least the last 7 years or two most recent employers.

Travel & Work Conditions

• Home visits with periodic office meetings.
• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not reimbursed).
• Local candidates accepted under the same terms as travelers.
• Some positions may have alternative shift schedules (e.g., 4x10s Friday-Monday), but most are standard Monday-Friday 5x8.

• Must be able to meet productivity expectations (approximately 32 weekly units).
