Description
Case Manager (Registered Nurse) - Seattle, Washington
Job Type
- Travel/Contract position
- Contract length: 13 weeks
- Shift: Day shift, 8:00 AM to 4:30 PM (5 days a week, 40 hours/week)
- Primarily onsite with occasional community visits to participant residences (mileage reimbursed)
- Locals accepted but not required
- Pending Washington state nursing license accepted
Job Description
- Provide comprehensive care management including patient assessment, care planning, implementation, coordination, monitoring, and ongoing evaluation
- Facilitate interdisciplinary team care coordination and intensive medical management for participants recently discharged from acute care or skilled nursing facilities
- Collaborate closely with social workers and physicians to gather medical and social information ensuring participant safety and well-being
- Work within an adult outpatient care program focused on elderly care, including clinics and home visits
- Assigned laptop and cell phone provided for work duties
Required Experience and Qualifications
- Valid Registered Nurse (RN) license in Washington
- Minimum 2 years of Acute Care Case Management experience, including experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Neonatal Resuscitation Program (NRP) certification
