• Monday to Friday, 8:00 AM to 5:00 PM (Day shift, 5x8 hours)

Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits with an average patient load of 5-8 patients per day.
• Types of visits include Start of Care, Wound Care, and Follow-up Visits.
• Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
• Willingness to travel up to 75 miles from the home clinic.
• Use of electronic medical records systems: HomeCare HomeBase (HCHB), OASIS, Outlook, and Microsoft Teams.
• Occasional on-call required (1-2 days per week, including some weekends).
• Local candidates accepted.

Required Qualifications

• Valid RN license in Tennessee.
• Minimum 1 year of recent Home Health experience, including experience with adult patients.
• Hospice experience preferred.
• BLS certification issued by the American Heart Association or American Red Cross.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Proficiency with HomeCare HomeBase within the last 6 months; if not current, retraining is required.
• Proficiency in OASIS.
• Experience working in a Home Health setting.
• Reliable transportation and valid driver’s license.
• Ability to meet mileage and travel requirements.

Additional Requirements

• Must complete various background checks including:

  • 7-year work and residence history verification.
  • State-specific criminal and abuse registry checks.
  • Drug screening (10-panel).
  • Fingerprinting as required by state.
• Health documentation required:

  • Physical exam within 12 months.
  • Negative TB test or chest X-ray within 12 months.
  • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
  • Seasonal flu vaccination or documented declination.
  • COVID-19 vaccination or documented declination.
• Completion and signing of compliance, confidentiality, privacy, and policy documents prior to start.
• Must review and complete home health-specific training and policy quizzes.
• Two professional references required.
• Verification of at least two most recent employers or last 7 years of work history.

Work Environment

• Dress code: Scrubs
• Patient visits are conducted in patients’ homes only.
• Schedule may vary slightly by location; some shifts may follow a 4x10 schedule (Friday-Monday) but most are standard 5x8.

Notes

• First-time travel nurses without recent specialty experience are not accepted.
• Weekend and on-call requirements vary; discuss during interview.
• Contract-to-hire possible based on performance.
