Occupational Therapist Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (13 weeks)
Shift: 08:00 AM – 04:30 PM

Job Summary

• Provide occupational therapy services to patients in their homes with cognitive and/or physical impairments due to injury, disease, disability, or other causes.
• Patient age range: birth to over 100 years.
• Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate services.
• Manage technical, environmental, and financial resources.
• Travel required within the geographic area.

Required Qualifications

• Licensed Occupational Therapist registered with the Massachusetts Board of Registration.
• Minimum 2 years of experience in acute care, rehabilitation, or home care as an Occupational Therapist.
• Ability to work with various computer software; familiarity with hand-held patient care devices preferred.
• Strong customer service skills and ability to work with diverse populations.
• Excellent verbal, written, and electronic communication skills.
• Ability to prioritize work and adapt to changing situations.

Required Documentation & Compliance

• Professional license/certification primary source verification (no conditional licenses accepted).
• Resume including past 7 years of work experience with city and state.
• Two professional references within the last 4 years (minimum one supervisor-level).
• Skills checklist completed within 1 year.
• Criminal background check, including Massachusetts CORI, national sex offender registry, and exclusion checks (OIG, GSA, Medicaid lists).
• Motor Vehicle Record (MVR) check (required for all home care positions).
• Education verification of highest degree achieved.
• Social Security Number trace covering last 7 years.
• Confirmation of Assignment/POI form upon acceptance and for extensions.
• Fit test for N95 respirator within 365 days of start date (no exemptions allowed).
• Respirator Medical Clearance attestation if fit test required.
• TB screening: documentation of negative TB skin test or blood test within 3 months of start date, updated if break >6 months.
• Health statement signed by a licensed provider within 365 days of start date, updated if break >6 months.
• MMR vaccination or proof of immunity.
• Tuberculosis symptom survey if positive TB test (optional).
• Hepatitis B vaccination or immunity proof (strongly preferred).
• Tdap vaccination within 10 years (preferred).
• Name change documentation if legal name differs from credential documents (optional).
• Submission coversheet required for initial assignment and new facility submissions.

Additional Notes

• No selfies or filtered photos allowed for badge photo submission; must be a recent, professional headshot.
• Background checks include international residence/travel over 30 consecutive days within last 7 years.
• All required documentation must be current and updated as specified to maintain compliance.
