Occupational Therapist Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (13 weeks)
Shift: 08:00 AM – 04:30 PM
Job Summary
- Provide occupational therapy services to patients in their homes with cognitive and/or physical impairments due to injury, disease, disability, or other causes.
- Patient age range: birth to over 100 years.
- Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate services.
- Manage technical, environmental, and financial resources.
- Travel required within the geographic area.
Required Qualifications
- Licensed Occupational Therapist registered with the Massachusetts Board of Registration.
- Minimum 2 years of experience in acute care, rehabilitation, or home care as an Occupational Therapist.
- Ability to work with various computer software; familiarity with hand-held patient care devices preferred.
- Strong customer service skills and ability to work with diverse populations.
- Excellent verbal, written, and electronic communication skills.
- Ability to prioritize work and adapt to changing situations.
Required Documentation & Compliance
- Professional license/certification primary source verification (no conditional licenses accepted).
- Resume including past 7 years of work experience with city and state.
- Two professional references within the last 4 years (minimum one supervisor-level).
- Skills checklist completed within 1 year.
- Criminal background check, including Massachusetts CORI, national sex offender registry, and exclusion checks (OIG, GSA, Medicaid lists).
- Motor Vehicle Record (MVR) check (required for all home care positions).
- Education verification of highest degree achieved.
- Social Security Number trace covering last 7 years.
- Confirmation of Assignment/POI form upon acceptance and for extensions.
- Fit test for N95 respirator within 365 days of start date (no exemptions allowed).
- Respirator Medical Clearance attestation if fit test required.
- TB screening: documentation of negative TB skin test or blood test within 3 months of start date, updated if break >6 months.
- Health statement signed by a licensed provider within 365 days of start date, updated if break >6 months.
- MMR vaccination or proof of immunity.
- Tuberculosis symptom survey if positive TB test (optional).
- Hepatitis B vaccination or immunity proof (strongly preferred).
- Tdap vaccination within 10 years (preferred).
- Name change documentation if legal name differs from credential documents (optional).
- Submission coversheet required for initial assignment and new facility submissions.
Additional Notes
- No selfies or filtered photos allowed for badge photo submission; must be a recent, professional headshot.
- Background checks include international residence/travel over 30 consecutive days within last 7 years.
- All required documentation must be current and updated as specified to maintain compliance.
