Occupational Therapist - Acute Care Hospital (Travel/Contract)

Location: Lafayette, Indiana
Contract Length: 13 weeks


Job Details

  • Work Schedule: Monday to Friday, plus one weekend day per month
  • Shift: Day shift, 8:00 AM to 4:30 PM (5 days/week, 8 hours/day)
  • Patient Population: Inpatient acute care rehab, including ICU to progressive care units
  • Common Diagnoses: CABG and other cardiac procedures, stroke, TBI, joint replacement, fracture repairs, respiratory illnesses, dementia, MS, Parkinson’s, ambulation issues
  • Responsibilities:
    • Evaluate patients and develop/follow plans of care (POC)
    • Provide ADL training, therapeutic exercises, and activities to improve function
    • Assess appropriateness of discharge disposition daily
  • Team: Part of a 19-member inpatient acute care rehab team
  • Charting System: Cerner
  • Dress Code: Royal Blue scrubs, black shoes

Required Qualifications

  • Valid Occupational Therapist license in Indiana
  • Minimum 1 year of Occupational Therapist experience in an acute care setting within the last year
  • BLS (Basic Life Support) certification

Compliance & Documentation Requirements

  • Valid Driver’s License or Passport (clear copy)
  • Background Check within 90 days of start, including:
    • 7-year residential/work history verification
    • SSN Trace, County Criminal, Statewide, National Sex Offender, FACIS Level III
    • Education Verification (may be pending at start)
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
  • Official Transcript or Diploma verifying degree and graduation date (may be pending at start)
  • Physical Exam within 12 months of start
  • Fit Mask Test within 12 months (not required for Occupational Therapists at this facility)
  • Immunizations and Titers:
    • Rubeola (Measles), Mumps, Rubella: Positive titer or 2 vaccines required; no declinations accepted
    • Varicella: Positive titer or 2 vaccines required; no declinations accepted
    • Hepatitis B: Series, titer, or signed declination required
    • Tetanus/Diphtheria/Tdap: Strongly recommended but not required
    • Flu Vaccination or approved medical/religious exemption required for flu season (9/30 - 3/31); declinations not accepted
  • Negative 10 Panel Drug Screen within 12 months of start (includes Amphetamines, Marijuana, Cocaine, Opiates, PCP)
  • TB Testing: Negative Tuberculin Skin Test (PPD), QuantiFERON-TB Gold, or T-SPOT within 1 year of start; if positive, chest X-ray and proof of past positive required
  • Color Blind Exam (Ishihara or equivalent) with 100% minimum score
  • Medication/Pharmacology Competency Exam required
  • Unit Competency Exam required
  • Skills Checklist completed within 12 months and annually thereafter
  • Minimum 2 professional references from recent employment verified within 2 years; references must be from charge, manager, supervisor, or above (team lead acceptable for Allied)
  • Current Employment Application covering last 7 years with explanations for gaps over 30 days
  • Completion of facility-specific compliance and confidentiality forms prior to start

Additional Notes

  • Indiana RN travelers are restricted from starting new contracts after 9/9/24 due to facility policy changes; exceptions apply for Perioperative/OR, Allied, and Ambulatory RN travelers.
  • On-site Fit Testing is limited and generally not available at this facility; PAPR use requires prior facility approval.
  • Orientation and policy acknowledgment packets must be completed prior to start.
