Job Opportunity: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Home Health

- Day shift, Monday through Friday
- Hours: 8:00 AM to 5:00 PM (5x8 hours)
- Some positions may have a 4x10 schedule (Friday-Monday), but most are standard 5x8
Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and other Licensed Practical/Vocational Nurses.
- Conduct home visits and periodic office meetings.
- Average patient load: 5-8 patients per day.
- Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
- Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
- Willingness to travel up to 75 miles from the home clinic; typical weekly travel around 150 miles.
- Visits may include both patient homes and care facilities.
Required Qualifications
- Valid LPN / LVN license in North Carolina
- Minimum 1 year of recent Home Health experience, including experience within the last year
- Experience working with adult patients in a Home Health setting
- Minimum 1 year Hospice experience
- BLS certification issued by the American Heart Association or American Red Cross
- Experience with HomeCare HomeBase (HCHB) EMR system
Additional Requirements
- Must complete various background checks, including:
- 7-year work and residence history
- State-specific criminal background and sex offender registry checks
- Fingerprinting as required by state regulations (NC SBI fingerprinting mandatory)
- Drug screening (10-panel)
- Health requirements include:
- Physical exam within 12 months prior to start
- Negative TB test or chest X-ray within 12 months
- Proof of immunizations or titers for Measles, Mumps, Rubella, Varicella, Hepatitis B
- Seasonal flu vaccination or documented declination
- COVID-19 vaccination series or documented declination
- Must review and sign various compliance, confidentiality, and policy documents prior to start.
- Two professional references required.
- Verification of at least two most recent employers or last 7 years of work history.
On-Call and Weekend Requirements
- On-call and weekend requirements vary by location and will be discussed during the interview.
Dress Code
- Scrubs
This position is ideal for experienced LVNs/LPNs with recent home health or hospice experience who are comfortable with travel and working independently in patient homes.
