Description

Community Hospice Case Manager (Nursing) - Colorado, Grand Junction

Job Type

Contract position
Full-time, 40 hours per week
Schedule: Monday to Friday, 8:00 AM to 5:00 PM
Occasional weekend backup and on-call duties

Job Description

Provide compassionate hospice nursing care in patients' homes, nursing homes, group homes, or assisted living facilities
Serve as a primary RN with an assigned caseload, leading the interdisciplinary care team
Act as a teacher to patients and caregivers, helping identify personal goals of care and building caregiver confidence
Manage symptom control and provide generalist care across all diseases, requiring strong assessment and critical thinking skills
Work in an environment that supports personalized, life-changing hospice care

Required Qualifications

Valid Registered Nurse (RN) license in Colorado
Minimum 2 years of case management experience, including experience within the last year
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Hospice nursing experience
