Physical Therapist (PT) - Home Health (Temp)

Location: Kāne‘ohe, Hawaii
Employment Type: Temporary
Shift: Days (8 hours per day)
Contract Length: 13 weeks

Job Description

• Plan and conduct treatment programs to restore and improve physical, social, and mental functions.
• Serve as co-leader in an interdisciplinary approach to patient groups.
• Complete evaluations to ensure quality standards and address assessment areas needing attention.
• Use appropriate clinical techniques and tools to establish individualized treatment plans.
• Provide therapy interventions following treatment protocols and techniques.
• Establish individualized care and discharge plans, ensuring communication to appropriate parties.
• Complete comprehensive and accurate clinical documentation within deadlines.
• Supervise assigned assistants.
• Perform other job-related duties as assigned.

Required Qualifications

• Minimum 2 years of Physical Therapist experience.
• At least 1 year of Physical Therapist home health experience.
• Valid Hawaii state Physical Therapist license.
• Bachelor’s degree required; Master’s degree preferred.
• Proficiency with Homecare Home Base and OASIS documentation systems.

Preferred Qualifications

• Prior travel experience as a Physical Therapist.

Productivity & Expectations

• Productivity expectation: 25 points per week.
• Adherence to organizational mission, vision, values, behavioral standards, policies, procedures, and Code of Conduct.
