Licensed Practical Nurse (LPN) - Home Health

Location: Agawam, Massachusetts
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and other Licensed Practical Nurses/Licensed Vocational Nurses.
• Average patient load: 5-8 patients per day.
• Types of visits include Start of Care, Wound Care, and Follow-up Visits.
• Must be willing to travel up to 75 miles from the home clinic, averaging about 200 miles per week.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hour shifts).
• No weekend shifts or on-call required for this position.
• Dress code: Scrubs.
• Charting system: Proficiency required in HomeCare HomeBase (HCHB) within the last 6 months; retraining provided if last use was over 6 months ago.

Required Qualifications

• Valid LPN/LVN license in Massachusetts
• Minimum of 1 year of LPN/LVN experience, including experience within the last year
• Experience working with adult patients in a Home Health setting
• Hospice experience
• BLS certification issued by the American Heart Association or American Red Cross
• Proficiency with HomeCare HomeBase (HCHB) electronic medical record system
