Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and other Licensed Practical/Vocational Nurses.
• Conduct home visits with an average patient load of 5-8 patients per day.
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
• Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
• Willingness to travel up to 75 miles from the home clinic; average travel about 150 miles per week.
• Use of electronic medical records systems HomeCare HomeBase (HCHB) and PointCare required.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours).
• Weekend and on-call requirements vary by location and must be discussed during the interview.
• Dress code: Scrubs.

Required Qualifications

• Valid LPN / LVN license in New Hampshire.
• Minimum 1 year of LPN / LVN experience within the last year.
• Experience working with adult patients in a Home Health setting.
• Hospice experience.
• Proficiency with HomeCare HomeBase (HCHB) and PointCare electronic medical records systems.
• BLS certification issued by the American Heart Association or American Red Cross.
