Physical Therapist Home Health - Travel/Contract
Location: Marlborough, Massachusetts
Contract Length: 13 weeks
Job Overview
- Provide physical therapy services in a home health setting with home visits and periodic office meetings.
- Average caseload: 5-10 patients per day (approximately 6-8 points daily, 30 points weekly).
- Types of cases: Start of Care and Follow Ups.
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5 days a week, 8 hours per day).
- Weekend rotation: None.
- On-call: None.
- Travel: Must be willing to travel up to 75 miles from home clinic, averaging 100-150 miles per week. Mileage reimbursed at IRS standard rate (patient-to-patient travel compensated; commute to/from first and last patient is not billable).
- Local candidates accepted at the same rate.
Required Qualifications
- Valid Physical Therapist license in Massachusetts.
- Minimum of 1 year of recent physical therapy experience within the last year.
- Experience working with adult patients in a home health setting.
- Current BLS certification.
Documentation and Compliance Requirements
- Must complete and maintain all required background checks, including:
- 7-year standard background check with SS trace, county, nationwide, sex offender, and state-specific searches.
- EPLS/GSA/SAM and HHS/OIG screenings.
- Fingerprinting as required by state regulations.
- Education verification.
- State-specific registry and clearance forms (e.g., Massachusetts CORI).
- Health documents required and must be current:
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months.
- Proof of immunity or vaccination for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- 10-panel drug screen.
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination per Massachusetts state policy (full vaccination required; no declinations accepted).
- Must complete facility-specific compliance documents and training prior to start, including but not limited to:
- Code of Conduct, Compliance Training Certification, HIPAA and Privacy Policy, Confidentiality Forms, Home Health Core Policy Acknowledgement, COVID-19 Policy and Attestation.
- Must sign new supplier packets at the start of each assignment and annually if working more than one consecutive year.
Additional Notes
- Patient census may vary; if hours are insufficient, discuss options with the agency for possible floating to nearby clinics.
- Weekend and on-call requirements vary by location; discuss during interview.
- The position covers specific towns in Massachusetts including Framingham, Natick, Ashland, Holliston, Millis, Sherborn, Dover, Needham, and Newton.
- EMR system used: HCHB.
