Registered Nurse - Rehab (Travel/Contract)

Location: Harvey, Illinois
Contract Length: 13 weeks
Shift: Day (3 shifts per week, 12 hours each; 6:15 AM - 6:45 PM)
Hours per Week: 36

Job Description

• Provide rehabilitation nursing care.

Required Qualifications

• Valid Registered Nurse (RN) license in Illinois.
• Minimum 1 year of rehabilitation nursing experience, with experience within the last year.
• Basic Life Support (BLS) certification issued by the American Heart Association.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).

Health & Safety Requirements

• Physical Exam: Within 1 year and updated annually, confirming good physical health.
• Fit Mask Test: Required within 1 year of start using approved masks (3M 1860 S/R, 1870+, or 9205+). Onsite fit testing available if needed.
• Immunizations/Vaccinations: Must provide proof or immunity documentation within 2 business days of contract signing for:

  • Tetanus/Diphtheria/Tdap (Td not accepted in lieu of Tdap)
  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (declination allowed)
  • Varicella (chickenpox)
• Tuberculosis Screening: Negative TB test (PPD, Qgold, or Tspot) within 1 year and updated annually; positive TB requires clear chest X-ray and questionnaire.
• Drug Testing: Negative 9-panel drug screen within 30 days of start (instant acceptable).
• Flu Vaccination: Required for flu season (10/1 to 3/31); religious or medical exemptions require facility approval. Declinations not accepted.

Documentation & Compliance

• Photo for Badge: Passport-style photo with plain background, forward-facing, no headgear.
• Orientation Modules: Must complete facility-specific orientation modules (~2 hours) prior to start.
• Fingerprinting: Required only if the position does not require an Illinois state license. Must be completed in Illinois and added to the Illinois Department of Public Health Registry.
• State License Verification: Primary source verification within 30 days of assignment start.
• NURSYS Verification: Required within 30 days of start for RN license.

Additional Notes

• COVID-19 and Influenza religious and medical exemption forms are available but must be submitted and approved at least one week prior to documentation due date.
• Prior travel experience is not explicitly required for this position.
