Physical Therapist - Skilled Nursing Facility (SNF)
Location: Seattle, Washington
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Setting: Long-Term Care (LTC) / Skilled Nursing Facility (SNF)
- Shift: Day shift, 8 hours per day (5x8 hour shifts, 8:00 AM - 4:30 PM)
- Patient Ratios: 1:15-25
- Weekend work: Not required
- On-call: Not required
- Floating between units: Not required
- Block schedules (e.g., 4x10s or 3x12s): Not accommodated
- Locals accepted within 50-75 mile radius
- Pending license accepted
- Guaranteed hours with possibility of up to one shift off per pay period
- Transportation: Traveler must have own transportation
- Attire: Black scrubs
- Charting system: Net Solutions
- Parking: Free
- Mileage reimbursement: IRS standard
Required Qualifications
- Valid Physical Therapist license in Washington
- Minimum 1 year of Physical Therapist experience, including experience within the last year
- Experience in a Long Term Care (LTC) setting
- Basic Life Support (BLS) certification required
Experience & Travel Requirements
- First-time travelers not accepted (prior travel experience required)
Documentation & Compliance Requirements
- Government-issued photo ID (Driver’s License or State ID)
- Social Security Card (as applicable)
- Background check within 90 days covering all locations lived/worked in past 7 years; must be updated annually
- Education verification required
- WA State Patrol Search (WATCH) within 90 days of start, updated every 2 years for SNF/LTC
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness to work
- Immunizations and health screenings:
- Tetanus/Diphtheria/Tdap (within 10 years or declination accepted)
- Negative TB test or chest X-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) vaccination or positive titer, declination accepted
- Varicella vaccination or positive titer, declination accepted
- Hepatitis B vaccination or signed declination
- Seasonal flu vaccine during flu season or declination
- COVID-19 vaccination record preferred; declinations accepted with adherence to masking policies
- Drug screening: 9-panel drug screen within 12 months of start
- Respiratory safety clearance required annually for travel candidates
- Physical exam and health documents required if onsite; not required if remote
Training & Competency
- Medication test/pharmacology competency exam as applicable
- Annual regulatory and specialty-specific training modules required via LMS
- Skills checklist updated annually
- Photo for badge (passport style with neutral background) required
Additional Notes
- Facility requires completion of a separate Washington State background check confirmation code within 72 hours of contract signing
- Float policy acknowledgment required (may be asked to float to other nearby facilities)
- References: Minimum 2 references covering 1 year of employment in a comparable specialty/setting within the last 3 years
- Employment application covering at least last 5 years required with explanations for any gaps or overlaps
