Job Opportunity: LPN, VNA Home Care
Location
- Northampton, Massachusetts
Employment Type
- Long-Term Assignment (approximately 13 weeks)
Schedule
- Monday to Friday, 8:00 AM – 4:30 PM
- One weekend shift required every 4 weeks (1 weekend rotation per month)
Job Description
- Provide skilled nursing care in patients' homes, performing approximately 6 patient visits per day.
- Work collaboratively within a multidisciplinary team including RNs, Physical Therapists, Occupational Therapists, and Home Health Aides.
- Deliver care as delegated by the RN Case Manager or Clinical Team Manager.
- Must have prior home care experience.
- Practice in accordance with the State Nurse Practice Act, professional standards, and applicable laws.
Required Qualifications
- Valid LPN / LVN license in Massachusetts
- Minimum of 1 year of Home Health experience within the last year
- Experience in a Home Care setting
- BLS certification issued by American Heart Association or Red Cross
