Job Description

• Position may float between units within the hospital.
• Weekends are required on a fluctuating basis.
• Block schedules are required.
• On-call is not required.

Required Qualifications

• Valid Physical Therapist license in Oregon

Health & Safety Requirements

• Physical exam within 1 year of start, signed by MD, NP, PA, or DO, confirming ability to work with or without accommodation.
• Fit mask test required annually (travel only; onsite completion possible).
• Immunizations and documentation required:

  • Tetanus/Diphtheria/Tdap (within 10 years; declination accepted)
  • Negative TB test or chest X-ray within 12 months (annual updates required)
  • Measles (Rubeola), Mumps, Rubella: 2 MMR vaccines or positive titer; declination accepted
  • Hepatitis B: vaccination series or signed declination; specific rules for Alaska placements
  • Varicella: 2 vaccines or positive titer; declination accepted
  • Seasonal Flu vaccine during flu season (declination accepted)
  • COVID vaccination record preferred; declinations not required but unvaccinated must follow infection control policies
• Drug screening: 9-panel drug screen within 12 months of start; instants acceptable; specific rules for Per Diem candidates.
• Respiratory safety clearance required annually for travel candidates.

Background & Verification

• Background check within 90 days of start covering all locations lived/worked in past 7 years; must be updated annually.
• Education verification required for all candidates.
• Employment application covering at least last 5 years of employment.
• Minimum of 2 professional references covering at least 1 year of employment in a comparable specialty/setting.
• Work history verification required if exceptions to references are requested.
• State-specific searches and registry checks as applicable (Oregon, Washington, Alaska).

Additional Requirements

• Photo for badge (passport style with neutral background) required.
• Completion of annual regulatory and specialty-specific training modules via LMS.
• Float policy acknowledgment required; traveler may be required to float to other nearby facilities.
• Competency exams and skills checklists must be updated annually as applicable.
