Job Summary

Provide direct skilled nursing care in the home setting using all phases of the nursing process, including assessment, diagnosis, planning, implementation, and evaluation to deliver patient care.

Key Responsibilities

• Coordinate patient and family care across all disciplines, including physicians, to meet physical, emotional, spiritual, and social needs.
• Follow physician-ordered plan of care and obtain additional orders as needed.
• Complete all required documentation timely according to department policy.
• Maintain a safe and risk-free environment for patients and their families/significant others.
• Participate in continuous quality improvement activities at agency and system levels.
• Collaborate with the healthcare team to implement policies, procedures, and quality improvement initiatives.
• Support orientation and partnership-building among healthcare team members.

Patient Load & Work Environment

• Patients per day: 5-8
• Travel radius: Calhoun County
• Electronic Medical Record system: McKesson

Required Qualifications

• Valid Registered Nurse (RN) license in Michigan
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Minimum 2 years of Home Health nursing experience, including experience within the last year
• Hospice nursing experience
• Basic Life Support (BLS) certification issued by the American Heart Association
