Occupational Therapist - Acute Care Hospital (Travel/Contract)

Location: Long Prairie, Minnesota
Contract Length: 13 weeks

Job Overview

• Work setting split: 60% hospital (general rehab), 40% outpatient (hands/upper extremity)
• Patient population: Adults of all ages
• Shift: Day shift, Monday to Friday, 8:00 AM – 4:30 PM (5x8 hour days)
• Weekly hours: 40 hours
• Traveler’s primary residence must be at least 25 miles from the facility

Required Qualifications

• Valid Occupational Therapist license in Minnesota
• Basic Life Support (BLS) certification
• Experience with Epic electronic health record system

Background and Compliance Requirements

• Criminal background check within one year of start date, including:

  • 7-year search, SS trace, counties, sex offender, nationwide, education verification
  • International background checks required if candidate has lived/worked outside the U.S.
• Minnesota DHS NetStudy Fingerprint clearance required before start (no start with fingerprints pending unless approved “more time needed” letter)
• Physical exam within one year of start
• Immunizations and health screenings required:

  • Tdap vaccination (no religious declinations accepted)
  • Negative TB test or chest x-ray per Minnesota state guidelines (2-step PPD or equivalent within 90 days of start)
  • Proof of immunity or vaccination for Measles, Mumps, Rubella, Varicella (MMR series or positive titers)
  • Hepatitis B vaccination, positive titer, or declination
  • Seasonal flu vaccination or documented declination (mask required if declined)
• Drug testing: 9-panel test within one year of start (instant acceptable)
• Two professional references from recent employers within 2 years
• Skills checklist required prior to start

Additional Facility Requirements

• Candidate must complete Minnesota DHS NetStudy Fingerprint Questionnaire immediately after signing contract
• Education modules must be completed prior to start for this location
• Dress code must be reviewed prior to start
• Photo for badge required (grey background, no selfies, JPG format)
• Confidentiality agreement must be signed before starting
• Candidates must return badges to manager on last day of assignment

Notes

• Background checks and fingerprinting have specific procedural steps and timelines; candidates must comply fully to avoid delays
• Facility does not require annual background check updates unless there is a gap of 30+ days between contracts
• Fingerprint registration expires after 2 weeks; candidates must complete fingerprinting within this timeframe or be re-registered
• Candidates may need to travel to a fingerprinting location if not within driving distance of a livescan site

This position is ideal for licensed Occupational Therapists with Minnesota OT licensure, BLS certification, and Epic experience who are comfortable working in both hospital and outpatient rehab settings.
