Licensed Practical Nurse (LPN) - Travel/Contract

Location: Wayland, New York
Contract Length: 26 weeks
Shift: Night (10 PM - 8 AM), 4 shifts per week (Wednesday to Friday)
Hours per Week: 40 hours

Job Description

• Provide nursing care to individuals with developmental disabilities under RN supervision.
• Administer medications and injections.
• Perform tube feedings and hang IVs.
• Collect blood samples if certified for venipuncture.
• Provide colostomy care.
• Monitor and report patient condition changes.
• Take and record vital signs (temperature, pulse, respiration, blood pressure).
• Obtain specimens for analysis and perform physician-ordered medical procedures.
• Prepare and apply simple dressings.
• Assist with bathing, feeding, and personal care.
• Perform light housekeeping tasks such as making beds, changing linens, and caring for personal effects.
• Must be available to stay beyond scheduled shift if another employee calls out, as patients cannot be left unattended by law.
• Required to float to other homes as needed based on schedule vacancies.

Required Qualifications

• Valid LPN / LVN license in New York.
• Minimum of 1 year of LPN / LVN experience, including experience within the last year.
• Experience in Long Term Care.

Background and Compliance Requirements

• Criminal background check fingerprinting required; processed onsite or internally depending on facility.
• Background check includes SS trace, county searches, nationwide search, and sex offender registry.
• Fingerprint Applicant Info and Consent forms must be completed and submitted as part of the process.
• Medication test/pharmacology competency exam required for LPN candidates (does not affect start date if pending).
• Completion of facility-specific Code of Conduct form and training certification required.

Additional Information

• This is a mandated facility under New York State law; patient safety requires continuous coverage.
• Floating to other homes may be required based on staffing needs.
