Job Opportunity: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Home Health

Location

Douglas, Georgia
Travel radius up to 75 miles from home clinic; must have reliable transportation and current driver's license.

Employment Type

Travel/Contract
Contract length: 13 weeks
Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hour shifts)
Weekend and on-call requirements vary; typically 1 weekend on-call per month rotation.

Job Description

Provide and coordinate home nursing care according to physician orders and agency policies.
Direct supervision and evaluation of Home Health Aides and other LVNs/LVNs.
Patient load: 5-8 patients per day, including start of care, wound care, and follow-up visits.
Visits primarily in patients' homes and some assisted living facilities.
Documentation and charting using HomeCare HomeBase (HCHB); proficiency required within the last 6 months or retraining provided.
Must be willing to travel within assigned territory (Atkinson, Bacon, Coffee counties).

Required Qualifications

Valid LPN / LVN license in Georgia.
Minimum 1 year of LPN / LVN experience within the last year.
Experience working with adult patients in a Home Health setting.
Minimum 1 year of hospice experience.
BLS certification issued by the American Heart Association or American Red Cross.
Experience with HomeCare HomeBase EMR system.
