Physical Therapist - Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (13 weeks)
Shift: 8:00 AM – 4:30 PM
Job Summary
Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. Patient age range is from birth to over 100 years. Collaborate with patients, families, physicians, and other health professionals to assess needs and coordinate care. Maintain thorough and timely electronic documentation and interdisciplinary communication according to agency policies and standards of practice.
Required Qualifications
- Valid Physical Therapist license in Massachusetts
- Minimum 1 year of Physical Therapist experience within the last year
- Experience in a Home Health setting
- Minimum 1 year of case management experience
Onboarding and Compliance Requirements
- Submission of a recent professional color photo (taken within the last 6 months, no selfies or filters, plain background).
- Motor Vehicle Record (MVR) check within 45 days of start.
- Primary source verification of professional license/certification within 30 days of start and updated upon expiration.
- N95 respirator fit test within 365 days of start, updated annually (no exemptions allowed).
- Nurse Aide Registry check within 30 days of start (required for all employees).
- Respirator Medical Clearance attestation if fit test is required.
- Documentation of immunity or vaccination for measles, mumps, and rubella (MMR).
- Tuberculosis (TB) test documentation within 3 months of start, updated if there is a break longer than 6 months.
- Physician health statement signed by MD/DO/NP/PA within 365 days of start, updated if >6 month break.
- Criminal background check within 45 days prior to start, including county and international residence/employment history for the last 7 years.
- Massachusetts CORI background check within 30 days prior to start.
- Submission of a detailed resume covering the past 7 years, including city and state for all work and education.
- Two professional references within the last 4 years, including at least one supervisor or higher.
- Completion of a skills checklist within 1 year of submission.
- Confirmation of assignment form required for all initial assignments and extensions.
- Education verification through background check company.
- Social Security Number trace as part of criminal background check.
- National sex offender registry check within 30 days of start, updated annually.
- Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists within 30 days of start and annually.
- Name change documentation if legal name differs from credential documentation.
- Candidate submission coversheet required for initial assignment submissions.
- Completion of attestation after registration in the applicant system.
Additional Notes
- Positive TB skin test requires documentation of a negative chest x-ray after the positive test.
- Weekend rotations are infrequent (once every 6 weeks).
- This role requires travel to patient homes as assigned.
