Seeking a Registered Nurse with case management experience to work in an acute hospital setting. The role involves utilization management, care coordination, discharge planning, and revenue integrity within a 36-bed unit.

Unit Type: Case Management
Number of Beds: 36
Number of Staff: 2
Shift Types: Day shift with weekend rotation, night shift
Patient Ratio: 18:1
Equipment: EMR system (Midas)

Required Qualifications

Valid RN license in New Hampshire
Case Management experience (minimum 2 years), including experience within the last year
Acute Hospital Case Management experience
BLS certification
Associate of Science in Nursing / Associate Degree in Nursing
Critical care experience
De-escalation skills in a healthcare setting

Work Hours

Approximately 36 hours per week
