Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Manage an average caseload of 5-8 patients per day.
- Types of visits include Start of Care, Wound Care, and Follow-up Visits.
- Work primarily hospice cases within the Alamance-Orange territory.
- Must be willing to travel up to 75 miles from the home clinic, averaging about 60 miles traveled per day.
- Schedule is typically Monday through Friday, 8:00 AM to 5:00 PM (5x8 hours).
- Some positions may have a 4x10 schedule (Friday-Monday), but most are standard 5x8.
- On-call requirement: approximately 1 day on-call; weekend requirements vary and should be discussed during the interview.
- Use of electronic medical records system HomeCare HomeBase (HCHB) and proficiency in OASIS required.
Required Qualifications
- Valid Registered Nurse (RN) license in North Carolina.
- Minimum of 1 year of hospice experience within the last year.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Experience working with adult patients.
- Experience in a home health setting.
- Proficiency with HomeCare HomeBase (HCHB) and OASIS.
- Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
