Medical Surgical LPN Travel/Contract Job - Rochester, NY
Position Details
- Job Title: Medical Surgical LPN
- Location: Rochester, New York
- Employment Type: Travel/Contract
- Contract Length: 13 weeks
- Shift: 3x12 hour shifts, rotating day (7a-7p) and night (7p-7a)
- Hours per Week: 36
- Weekend Requirement: Every other weekend
- Holiday Requirement: Available to work every other holiday, if needed
- Charting System: EPIC experience required
- Floating: Required to float within skillset
- Patient Ratios: 1:5–6
- Unit Description: Complex care unit for medicine, orthopedic overflow, and complex long-term patients
- Scrub Color: Navy blue and white combinations
Required Qualifications
Licenses & Certifications
- Valid LPN / LVN license in New York
- Basic Life Support (BLS) certification issued by American Heart Association
Experience
- Minimum of 2 years LPN / LVN experience, including experience within the last year
- Medical Surgical (Med Surg) experience required
- Telemetry experience
- Experience with EPIC charting system
Compliance & Onboarding Requirements
Background Checks & Verifications
- 7-year criminal background check (felony checks) within 3 months of start
- Social Security Number trace, national criminal, sex offender registry, education verification from primary source
- EPLS/GSA/SAM and HHS/OIG checks within 3 months of start and monthly thereafter
- Foreign equivalency education evaluation if applicable
- Employment application and work history verification required
- References required for some candidates (2 references within 3 years)
Health & Immunization Requirements (Must be completed within 5 days of contract signing)
- Physical exam within 1 year, updated annually
- Fit mask test within 1 year, updated annually (specific mask types accepted)
- Negative TB test or chest X-ray within 1 year; TB questionnaire accepted in lieu of annual renewal
- Immunizations or positive titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (surface antibody positive titer, completed series, or declination)
- Varicella
- Drug screening within 30 days of start (includes amphetamines, cocaine, fentanyl, heroin, opiates, etc.)
- Seasonal flu vaccination or signed declination during flu season
- Snellen eye exam within 1 year (not required annually)
- COVID vaccination copy preferred but not required
Facility-Specific Documents
- Government-issued photo ID required
- Passport-style photo for badge
- Various attestations and forms to be completed prior to or during onboarding
- Parking pass form required for orientation
Additional Notes
- The client may cancel up to three shifts during the contract without penalty
- Requests for time off, block, and special scheduling must be submitted upon application
- Locals accepted with adjusted pay rate
- Magnet-recognized acute care teaching hospital with advanced stroke center certification and multiple specialty programs
