Description

Licensed Practical Nurse (LPN) - Part Time PM Shift

Location: New Lisbon, Wisconsin
Employment Type: Contract
Contract Length: 13 weeks
Shift: Part-time, PM Shift (2pm-10pm), every other weekend
Hours per Week: 34 hours

Job Summary

Assess and administer medications to residents following the 5 Rights.
Report any changes in resident condition to the charge nurse and update physicians and family members as needed.
Report any allegations of abuse, neglect, or misappropriation to appropriate leadership.
Assist CNAs with call lights and other duties.
Perform other duties as requested.

Required Qualifications

Valid LPN/LVN license in Wisconsin.
Minimum of 1 year of Long Term Care experience within the last year.
Current BLS certification.

Additional Information

Open to candidates with RN or LPN licensure.
