Description
Registered Nurse - Case Manager (Travel)
Location: Denver, Colorado
Employment Type: Travel
Contract Length: 13 weeks
Hours per Week: 36 hours
Unit Information
- Unit: Case Management
- Typical Unit Size: Approximately 20 beds per unit
- Patient Ratios:
- RN: 1:20
- Social Worker: 1:40
- Team: Case Managers and Social Workers collaborate to manage patient load
- Equipment: Mobile devices, computers
- Electronic Medical Records (EMR) systems used: Meditech, Midas, Navihealth
Schedule
- Day Shift Options:
- Friday to Monday, 7:30 AM to 5:30 PM
- Monday to Friday, 8:00 AM to 4:30 PM
- Some weekends may be required
Required Qualifications
- Valid Registered Nurse (RN) license in Colorado
- Minimum 1 year of acute care Case Management experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- BLS certification
Preferred Qualifications
- Emergency Department (ED) experience
- Knowledge of Palliative Care and Goals of Care (GOC) conversations
Typical Responsibilities
- Suicide risk assessments
- SBIRT (Screening, Brief Intervention, and Referral to Treatment) / Substance use assessments
- Discharge planning assessments and coordination
- Behavioral health experience, particularly with suicide risk and substance use assessments
