Registered Nurse - Case Manager (Travel)

Location: Denver, Colorado
Employment Type: Travel
Contract Length: 13 weeks
Hours per Week: 36 hours

Unit Information

• Unit: Case Management
• Typical Unit Size: Approximately 20 beds per unit
• Patient Ratios:

  • RN: 1:20
  • Social Worker: 1:40
• Team: Case Managers and Social Workers collaborate to manage patient load
• Equipment: Mobile devices, computers
• Electronic Medical Records (EMR) systems used: Meditech, Midas, Navihealth

Schedule

• Day Shift Options:

  • Friday to Monday, 7:30 AM to 5:30 PM
  • Monday to Friday, 8:00 AM to 4:30 PM
• Some weekends may be required

Required Qualifications

• Valid Registered Nurse (RN) license in Colorado
• Minimum 1 year of acute care Case Management experience within the last year
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• BLS certification

Preferred Qualifications

• Emergency Department (ED) experience
• Knowledge of Palliative Care and Goals of Care (GOC) conversations

Typical Responsibilities

• Suicide risk assessments
• SBIRT (Screening, Brief Intervention, and Referral to Treatment) / Substance use assessments
• Discharge planning assessments and coordination
• Behavioral health experience, particularly with suicide risk and substance use assessments
