Job Description
- Outpatient infusion pharmacist role with oversight of medication use process in outpatient infusion center
- Order verification of non-hazardous and hazardous outpatient infusion medications, including oncology
- IV infusion compounding product evaluation and verification
- Evaluation of oncology and non-oncology treatment plans
- Verification of insurance coverage prior to treatment initiation
- Patient education on infusion medications
- Participation in management of infusion adverse reactions
- Expected patient ratio: 1:20
Required Qualifications
- Valid Pharmacist license in Oregon
- Minimum 1 year of pharmacist experience within the last year
- Experience in an outpatient setting
Background and Compliance Requirements
- Government-issued photo ID (Driver’s License or State ID)
- Social Security Card (as applicable)
- Background check within 90 days covering all locations lived/worked in past 7 years; updated annually
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start; updated monthly
- Education verification required
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Drug screening (9 panel) within 12 months of start
- Immunizations and health documents:
- TDAP within 10 years (declination accepted)
- Negative TB test or chest X-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal flu vaccine during flu season (declination accepted)
- COVID vaccination record preferred (declinations not required)
- Respiratory safety clearance required annually for travel candidates
- Fit mask testing required annually for travel candidates (onsite or valid record)
- References: Minimum 2 references covering 1 year of employment within last 3 years in comparable specialty/setting
- Employment application covering at least last 5 years of employment

- Passport-style badge photo with neutral background
- Completion of annual regulatory and specialty-specific training modules via LMS
- Competency exams and skills checklists as applicable
- Compliance with state-specific background and licensing requirements
