Required Qualifications

• Valid Polysomnographer license in Oregon
• Minimum of 1 year of Polysomnographer experience
• Polysomnographer experience within the last year

Orientation and Scheduling

• Orientation: Reduced Non-Billable Orientation (0 hours vs. standard 40 hours)
• 48-hour shifts: Not approved

Background and Compliance Requirements

• Background Check:

  • Must be within 90 days of start date covering all locations lived/worked in past 7 years
  • Includes SSN Trace, Death Master Index, Nationwide and County checks, Sex Offender Registry, FACIS Level 3, Education Verification, MVR (if applicable)
  • Annual updates required
  • Specific Oregon Abuse/Neglect search required only for certain facilities (not applicable here)
• E-Verify: Required
• Education Verification: Required for all candidates
• References: Minimum 2 references covering 1 year of employment within last 3 years in comparable specialty/setting

Health and Safety Requirements

• Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO, confirming ability to work with or without accommodation
• Fit Mask Test: Required for travel candidates, valid within 1 year, updated annually
• Immunizations and Testing:

  • TDAP vaccine within 10 years or signed declination
  • Negative TB test or chest X-ray within 12 months, updated annually
  • MMR (Measles, Mumps, Rubella): 2 vaccines or positive titer or signed declination
  • Hepatitis B: Documentation of vaccination series or signed declination
  • Varicella: 2 vaccines or positive titer or signed declination
  • Flu vaccine during flu season or signed declination
  • COVID vaccination record preferred; declinations accepted with adherence to infection control policies
• Drug Testing: 9-panel drug screen within 12 months of start (instant acceptable)

Competency and Training Requirements

• Annual Regulatory and Specialty-Specific Modules: Required annually via LMS (Workramp)
• Other Competencies: Color Blind Testing if required by unit
