Registered Nurse - Home Health (South Carolina)

Job Details

• Location: Aiken, South Carolina
• Employment Type: Travel/Contract
• Contract Length: 13 weeks
• Shift: Day shift, Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours)
• Weekly Hours: 40 hours
• Average Patients per Day: 5-8
• Patient Visits: 5-10 patients per day, including Start of Care, Wound Care, Follow-up Visits
• Travel: Must be willing to travel up to 75 miles from home clinic; estimated 200-350 miles per week traveling patient to patient
• Schedule: Mostly Monday-Friday; weekend rotation and on-call requirements vary and should be discussed during interview
• Guaranteed Hours: No guaranteed hours; possibility to float to nearby clinic if patient census is low
• Charting System: Proficiency required in HomeCare HomeBase (HCHB) and OASIS (for RNs) within the last 6 months; retraining required if last use was over 6 months ago
• Dress Code: Scrubs

Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
• Responsible for documentation, travel, and patient care activities measured by a points system (30 points per week expected)

Required Qualifications

• Valid RN license in South Carolina
• Associate of Science in Nursing / Associate Degree in Nursing
• Minimum 1 year of recent Home Health experience working with adult patients within the last year
• Current BLS certification issued by the American Heart Association or American Red Cross
• Experience with HomeCare HomeBase (HCHB) and OASIS documentation systems
• Valid driver’s license and reliable transportation

Additional Requirements & Compliance

• Must complete various background checks including 7-year history, sex offender, state-specific searches, and fingerprinting if not resident in SC for 12 months prior to start
• Drug screening: 10-panel drug test required
• Health documents required: Physical exam within 12 months, negative TB test or chest X-ray within 12 months, proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella, and seasonal flu vaccination or declination
• Must review and sign multiple compliance and policy documents prior to start, including confidentiality, HIPAA, privacy policies, and home health core policies
• References: Two professional references required
• Work history verification: Must verify at least two most recent employers or last 7 years if only one employer

Notes

• First-time travelers are not accepted; recent specialty experience is mandatory
• On-call and weekend requirements vary by location and should be clarified during interview
• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not reimbursed)

This position is ideal for experienced RNs with recent home health or hospice experience who are comfortable with travel and independent patient care in a home setting.
