Description

Registered Nurse - Hospice (Travel/Contract)

Location: Windham, New Hampshire
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (5x8 hours)
Hours per Week: 40


Job Description

  • Provide and coordinate home nursing care according to physician orders and agency policies.
  • Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Conduct home visits with an average patient load of 5-8 patients per day.
  • Types of visits include Start of Care, Wound Care, and Follow-up Visits.
  • Must be willing to travel up to 75 miles from the home clinic, covering approximately 200 miles per week.
  • Documentation and charting are done using HomeCare HomeBase (HCHB); proficiency required within the last 6 months. RNs must also be proficient in OASIS.
  • Dress code: Scrubs.
  • Weekend and on-call requirements vary; discuss during interview.
  • Some positions may have a 4x10 schedule (Friday-Monday), but most are standard 5x8 Monday-Friday.
  • Local candidates accepted at the same rate.

Required Qualifications

  • Valid RN license in New Hampshire.
  • Minimum 1 year of hospice nursing experience within the last year.
  • BLS certification issued by the American Heart Association or American Red Cross.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Experience working with adult patients.
  • Experience in a home health setting.
  • Proficiency with HomeCare HomeBase (HCHB) documentation system.

Productivity Expectations

  • Weekly productivity units: 25-30 units.
  • Patient assignments typically equate to 6-8 points per day based on visit types and activities.

Compliance and Documentation Requirements

Candidates must complete and maintain the following prior to and during assignment:

  • Background checks covering the last 7 years, including SS trace, county, nationwide, sex offender, and state-specific searches.
  • Drug screening (10-panel).
  • Physical exam within 12 months confirming good health and no communicable diseases.
  • Negative TB test or chest X-ray within 12 months.
  • Immunization or titer proof for Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella, and seasonal flu vaccination or declination.
  • COVID-19 vaccination series or declination.
  • Various state-specific releases and fingerprinting as required by the assignment location.
  • Completion of agency-required compliance training, policy acknowledgments, confidentiality agreements, and quizzes related to hospice care.
  • Proof of personal or auto insurance valid and under candidate’s name or listing candidate as a driver.

Additional Notes

  • On-call and weekend requirements vary by location and should be clarified during the interview.
  • No guaranteed hours; if patient census is low, floating to a nearby clinic may be discussed.
  • Candidates must sign new supplier packets at the start of each assignment and annually if working consecutively for more than one year.
  • Must be willing to complete all required compliance and facility-specific documents prior to start.

This position is ideal for experienced hospice RNs with recent specialty experience who are comfortable with travel and home health care documentation systems.
