Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Directly supervise and evaluate Home Health Aides and other Licensed Practical/Vocational Nurses.
- Conduct home visits with an average patient load of 5-8 patients per day.
- Types of visits include Start of Care, Wound Care, and Follow-up Visits.
- Must be willing to travel up to 75 miles from the home clinic, averaging 150-200 miles per week.
- Schedule is typically Monday through Friday, 8:00 AM to 5:00 PM (5x8 hours).
- Weekend and on-call requirements vary and should be discussed during the interview.
- Use of electronic medical records system HomeCare HomeBase (HCHB) required; proficiency within the last 6 months preferred.
- Home health experience with OASIS documentation is required.
Required Qualifications
- Valid LPN/LVN license in Virginia.
- Minimum of 1 year of recent experience in Home Health nursing, including experience within the last year.
- Experience working with adult patients in a Home Health setting.
- Hospice experience.
- Current Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
- Proficiency with HomeCare HomeBase (HCHB) electronic medical records system.
- Valid driver's license.
Additional Requirements & Compliance
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella, and seasonal flu vaccination or declination.
- 10-panel drug screen required.
- Completion of various compliance documents and training prior to start, including confidentiality, HIPAA, privacy policies, and home health core policies.
- Background checks covering the last 7 years, including state and federal searches, sex offender registry, and other state-specific requirements.
- Two professional references required.
- Work history verification for at least the last 7 years or two most recent employers.
Work Environment & Other Details
- Dress code: Scrubs.
- No guaranteed hours; patient census may vary.
- Floating to nearby clinics may be available if patient load is low.
- Local candidates accepted under the same terms as travelers.
- Some positions may have alternative shift schedules (e.g., 4x10s Friday-Monday), but most are standard 5x8 Monday-Friday.
This position is ideal for experienced LVN/LPNs with recent home health or hospice experience who are comfortable with travel and working independently in patients' homes.
