Description

Primary RN - Case Manager (Community Hospice)

Location: Grand Junction, Colorado
Employment Type: Contract
Contract Length: 6 weeks

Job Description

Provide compassionate hospice nursing care, supporting patients and families with strength and courage.
Serve as a primary RN with an assigned caseload, leading the interdisciplinary team for those patients.
Deliver care in various settings including private homes, nursing homes, group homes, and assisted living facilities.
Act as a teacher to patients and caregivers, helping them identify personal goals of care and build confidence in caregiving.
Manage symptom control and perform comprehensive assessments requiring critical thinking.
Work schedule is Monday to Friday, 8 AM to 5 PM, with occasional weekend backup and on-call duties.

Required Qualifications

Valid RN license in Colorado
Minimum of 2 years of case management experience, including experience within the last year
Current BLS certification
Associate of Science in Nursing or Associate Degree in Nursing
Hospice nursing experience

Additional Information

This role involves community-based hospice care with a focus on personalized patient-centered service.
Prior travel experience is not indicated as a requirement.
